I have a pair of white spotted Pygmy filefish in my 34 gallon sps tank and I'm wanting to add a mystery wrasse. I'm hesitant to do so as I've heard they can be aggressive.

Thoughts?

Mystery wrasse and six line are terror fish by and large. There are exceptions but as a general rule, avoid them like the plague like damsels IMO.

I have seen a 1.5" six line bully and kill a 5" clown tang.

^Agreed, mystery wrasses can be a problem, especially the longer they are in a tank. They really develop a dislike for fish added after them, although I don't think it will be a problem with the filefish.

Mystery wrasses also get bigger than most of their congenerics, and will eventually outgrow that sized tank.

I have a well-established 220 with some large aggressive fish that generally get along well. Two triggers (had for 7 and 13 years), two angels (adult emperor and lemon peel), powder blue, naso, snowflake clown, and a somewhat recently added six line.

I now have a mystery in QT. Do you guys think I'll be ok?

The sixline and the mystery are not likely to get along well, if at all.

Even in 220 gallons? My six line seems pretty timid amongst all of my big bullies.
Unfortunately, yes. It might be okay, but that's not the result I would expect.

I'm most worried about my Lemmon peel. She's a mean one and chased the six line for many days when I first put it in.
It may get bullied by some of the bigger, more aggressive fish, but the biggest issues will be between the sixline and the mystery wrasses. The greatest liklihood is that they will fight.

Will mystery wrasse pick on feather dusters or sea apples?

Those are usually ok, but wrasses can be quite variable in behavior.
