I think it's a Dragon Soul Goniastrea but it's looking a bit rough.

I'd try to gently blow off the algae from the exposed skeleton and make sure it's not being bothered or picked at by anything. What are your parameters? Lighting?

Par is 94 and the nitrates are 10ppm
