Phosphate & Calcium

I'm looking for some info on the above.

1. What causes it?
2. What levels should you have?
3. How to fix it?

Perhaps it would lead to a better answer if you explained why you are asking. What are your levels? Is that the concern?

The range of both that is OK is fairly large. I'd target 0.01 to 0.05 ppm phosphate and 400-550 ppm calcium.
