Okay I’m very confused on what is going on so all corals and one other nem (kept very far away so could have not been stung and is also a bta) no sudden changes in water or temp being 88.5 kept at levels at 9 alk 450 calc and 1350 mag and kept constant with a Neptune trident. Could this nem be infected or something also I don’t have clowns so that can’t be it ever it has been doing good I don’t know why it looks dead now it’s been in here for months

How long has tank been setup for? Also would suggest lowering temp by 10 degrees. Maybe that was a typo?

If your parameters as within range, I wouldn't worry. I ve seen my 1ft wide BTA shrink to the size of a quarter before...

It doesn’t seem to improve in the last few weeks though and much worse today

That nem doesn't look good at all, honestly it's not likely you'll save it ime. How did it look when you got it? Yes it could have been sick or gotten injured did you check it for injury when you got it? Some places are horrible at removing them and damage the foot. I've had good results using chloramphenicol for nems in the case of infection etc.
