Reoccurring red cyano

I used chemi clean about a year ago on my 125g and it wiped out my cyano but it started to come back so clearly the problem is not solved. The tank is about 3 years old. 6 fish, 2 large and 4 smaller fish. I use fritz rpm salt( the red box). I rarely do water changes though. Any ideas on how to solve my cyano problem.

Perhaps you probably should be doing more water changes if you want to combat this problem

Last time I had this problem water changes seemed to accelerate the problem. Im going to do a water change today. I havent test in a long time because softies have high tolerance.
 
The glass is green and there is a lot of debris flying around in the tank, and not doing water changes or any testing kind of hints to a lack of maintenance. If you're only doing maintenance when a problem comes up, then thats why the problem comes up.

Yea that's probably true. The debris is because I was cleaning the tank getting ready for a water change but there is a lot of algae on the glass.
