Sand keeps killing Acans

Hi everyone! I’m extremely confused as to what is going on with my Acans. For years I have not been able to successfully grow out a colony. I keep the frag for weeks. It grows and looks amazing! Until one day I check my tank to see sand stuck in the mouth of a polyp. Ill clear the sand out but day after day it keeps coming back and takes a polyp out one by one until the frag is almost completely dead then it will make a come back from one polyp. This is extremely frustrating and confusing because this is the only coral this happens to. The rest of the tank is healthy and thriving. I just don’t know how to keep sand out of my Acans or what keeps putting sand in them! Current fish are just 2 clowns but this has happened far before they were in the tank. Any input or help would be greatly appreciated to help save my coral.

What sort of flow do you have going? Is there any shrimp in there that dig in the sand? I have seen my female clown kick up sand with her tail.
 
I suck with acans and it's not sand that's the problem.

I have one power head going it’s perfect to keep torches and monti at the top of the tank and softies at the bottom nothing crazy going on. As for shrimp or inverts I have nothing but 2 snails and a few hermit crabs. It’s only my acans specifically this happens to. I think it’s very strange
