Toadstool not looking good

I went to my LFS and picked up a toadstool and a few days later it’s not looking good. Is it dead? And should I remove it from the tank
I’m running two radions xr15 on readsea reefer 350 g2

Also I’m new to reefing and any info or tips is greatly appreciated!!!
