Torch and favia help!

my torch hasn't opened in about 3 days, my favia has turned from green to brown. Dipped into revive and little tiny worm like things came off. About the size of the comma on a keyboard. , favia has a white jelly in the middle. Do I need to remove them or do they have a chance.

Salinity .023
Cal 450
Nitrate 20ish

Please help

Looks like that torch is a goner, especially the left head. What are your actual parameters phosphate and Alk?Tank size? Lighting? Age of tank? Do you always run your salinity that low? Need more details. Also looks like the zoanthids aren’t happy.
