Torch coral dying?

I have a few torches that are doing good at all and can't figure out what's going on. All my parameters seem to be alright, not perfect but dont think it would cause this, could be wrong. I thought it might have been a light a flow issue so I moved them to more suitable location and it seem to help most of the heads except this one. This morning I noticed most of the white flesh on the septa( sorry if that's the wrong terminology) is almost gone in the past 48 hours or so.

No read on the nutrients?
Low or no or unstable N and P can easily make corals starve, or worse, set up an environment which fosters pest stuff.

Check those first.

Btw…..torches hate hate hate to be moved.
 
Im currently out of hanna reagents for both. I'm going to grab salifert tests at my local coral store. Last I tested my nitrate was at 15ppm and the Phosphates were at .1, I added 2 new clowns to the tank so both slowly go up a bit. Everything else seems to be doing good.
 
For anyone wondering I'm pretty sure I've figured it out. After watching and observing and trying a couple subtle changes with only a negative impact, I started looking even closer and I observed what I believe to be spionid worms. When I first got them they stared as 2 large branches with 5 heads on one, and 5 on the other, with very small flesh bands. While I was trying to place one it broke abnormally easily. Since that day I have noticed most of them slowly losing flesh and suffering from polyp bailout and or tentacles randomly falling off. The heads with little to no worms have also been seeming to do better then the others, even split heads on the same branch.

Just fragged all the heads off about half inch above what I could see for antennae coming from the stock. Going to continue observing with no changes to any of my parameters and see what happens.
