Getting lazier and lazier with upkeep!
Looks like cyano to me. Siphon out as much as possible to start out.
Stabilize your parameters and ensure neither nitrates or phosphates are zero while the other nutrient is present.
If that doesn't do it in a few months, then attempt chemi-clean.
