Chunk missing from flame angel. Dead.

A few days ago i noticed my flame angel was not swimming around the tank and was hiding.
The next morning i found him dead hidden in the rocks.
Once i pulled him out i saw a massive gash on his side but what could have done this?

Tank inhabitants are: 1 fox face, 2 clowns, 2 green chromis, 1 coral beauty.
I think it was the coral beauty but she is much smaller than he was and they didnt really show much aggression towards eachother. Could the coral beauty of made a gash like that on him?
Tank size is 200 gallons with lots of hiding places.

Hard to say but there's a good chance the gash wasn't there when it died... critters that eat dead stuff may have made it.
The gash was there when he was alive the night before, but i could not get him out. it might not of been as large though. was very hard to see when he was in the rocks.

It looks to me like an ulcer was forming, this is most commonly formed by stress or poor water conditions.
