As in your pic, they will deflate occasionally But do monitor your water. Clowns are notorious for pounding the daylights out of a nem to this state.
Check Nitrates, ammonia and Ph level. Moderate light and water flow is all they ask for.

Could be too much clown for the nem size. What kind is it? Hard to tell if BTA or Ritteri.
