Tank age and water parameters?
What happened recently in your tank that was different?

That ricordea mushroom is most likely the culprit. I always gave my ricordea mushrooms their own rock. Corals will fight for space and softies can use a defense known as allepothey. They also have nematocysts that potentially sting as well.

I've kept Duncans ... they can be difficult coral to keep.
My tank is pushing 6yrs old, i have a frag added 8 months ago and its still the same - stubborn little coral.
