@Jay Hemdal any advice on this? The fish is still eating. Sometimes he will come off the bottom and swim circles around the QT tank, bumping sides at times but also sometimes it seems like he either knows where he's going or he's memorized the tank. Also, he will spin in circles. Still eats but I have to spot feed to get him any food. Eyes still twitching as shown in the previous video

Sorry - I missed your 2/18 post. It does sound like permanent blindness. I'm not sure if it will adapt to the display tank or not. It's probably able to find food easier in a bare bottom tank.

You could try the Maracyn, but I just don't see signs that this is an external bacterial infection.

My powder blue tang went blind back in February and had the eyes twitching/blindness as shown in the video posted. My tank went fallow for 2 months and I put all occupants through your standard quarantine protocol. I just recently reintroduced them back into the display after a successful treatment and the same blindness occurred in my coral beauty, same eye twitching as the powder blue. Any ideas what could be causing this? There are 0 signs of ich or any other diseases.

I didn't get pictures as I took off for vacation this morning.

My Parameters are normal, the only thing I'm dosing is calcium hydroxide in my ATO, AFR, and magnesium.

This is bizzare. Have you done a manual test on nitrate to assure not false readings?
Some causes are poor/inadequate nutrition, collision with objects, High nitrates, even ammonia, very bright lighting, cataract issues often water related and vitamin A deficiency.
I would recommend getting an ICP test done

Are you making your own water? Is it possible there is something in the water leading to blindness? Lead or something?

Lights can't be too bright, parnis max 350 near the surface of the tank. Parameters weren't great with icp but I've taken corrective action by dosing.

Tests used are hanna for alk, nitrate and phosphates. Salifert for mag and calcium.

Attached are the ati results from late march.

The only abnormal behavior I've noticed is my foxface swims back and forth pretty fast at times. Almost frantically, and started doing this during quarantine. If I look through the front glass I can see the side panes almost act as mirrors, so maybe it's getting spooked?

Sorry - I’m not sure what the causes of multiple cases of blindness might be. As I mentioned, this looks neurological. Nematodes and coccidea are internal parasites that can cause neurological damage, but usually they hit different nerves, so the symptoms are not exactly identical from fish to fish.
