Blue Tang help

I recently bought a 75 gallon salt watertank(already have 35 gallon). Anyways when the couple I bought it from started to neglect it. I'm wondering if the blue tang has ICH, MHLLE or both.
No other fish seem to have any issues, but the tang is pretty beat up.
Help!!!

Welcome to Reef2Reef! Sorry about the tang :(

I'm not the best at fish diseases, so my first inclination is to say both ich and MHLLE, but hopefully a real expert will come along and set the story straight.

I see Velvet, and big time HLLE. Start with freshwater dip. This will rid of any flukes and calm down parasite.
In separate bucket at same tank temperature, give it formalin Bath (quick cure would be a good form of formalin).
Huge vitamin deficiency !! Feed it :

Nori seaweed basted with Garlic
Formula 2 flake and frozen
LRS Herbivore diet
Spirulina brine shrimp
Mysis shrimp
Small plankton

Do add Selcon or similar vitamins to food 2-3X per week
