Bristleworm in Deresa Clam ??

Hello,
I have had this clam for almost a year and a half. Around mid October I had an alkalinity swing, and have since gotten it in check. Since that has opened the clam has been opening up less, and even now it doesn’t open as much after having stable levels for a little while. I was researching and I saw something about pyramid snails, so I looked under to find a bristeworm partially sticking out. I have been stumped on why it isn’t doing well, and I don’t know if this is enough cause for it to remain closed.

Also I don’t have a photo of it recently when it was more closed, but the mantle was about half extended compared to the above photo, and the clam itself was about half as wide open
