Bubble tip anemone help!

Hello everyone I’m trying to figure out what could be wrong with my bubble tip anemone I’ve had him for about a month now and he was doing really good but lately hasn’t been coming out and has been constantly sucked up. Water chemistry is good I’ve been testing every few days since he’s been looking ugly and they have been stable I contacted a lfs and told them about it and they say it could be an infection but not sure. I’m gonna add some pictures and show you guys and hopefully someone has and idea of what’s going wrong or what should do I’m currently setting up a quarantine tank in case it is and infection. Also he does come out of his hole and he is eating and getting his light for a couple hours but just want always wants to go back into hiding

Unfortunately the picture do not show clearly enough what is happening. Some people have used antibiotics in situations like this (in a separate quarantine tank). Are you sure your lighting, etc - is optimal - is it similar to the tank from which it came? Are your parameters ok? Does it ever come out looking totally normal - or always a little 'poor'. is there anything near the anemone that could be causing a problem

My light is 12 inch ai blade grow I have Zoas hammers and a torch and every else is doing really good beside him. I was thinking about getting a wave maker and seeing how he likes that but when I turn the heads that come in a 13.5 evo tank he seems to not like the flow so still debating it. Water parameters have been great I’m getting a magnesium testing kit this weekend to make sure that is in line as well. also the lfs I purchased from had there water colder than what I keep mine at but like I said he was doing really well the first couple weeks. I also should mention I had a pistol shrimp that put some sand around him but he has been removed since

It doesn't look terrible from the picture. I think I'd be more concerned if the tentacles were all hanging out and shriveled up.
How long are your lights on?
Also, what do you mean about eating? Are you direct feeding or is it just getting stuff floating by when you feed your fish?
I've found for my RBTA's that they like it when I feed them a chunk of frozen food about once a week.

I talked to my guy at my lfs and reassured me he’s more than likely just agitated I feel like I may have overreacted with the quarantine tank and everything lol but I feed him a mix of bene reef and some frozen brine and he likes it also have been dosing with plankton for the other corals in my tank
