Can't seem to keep LPS alive, need advice

Just can't seem to keep these going. Not sure whats going on. Top hammer is the newest, but not by much, bottom two seem on their way out. Anything I can do to improve? Parameters are stable, no swings. I did have a hammer get brown jelly last month.

How's the flow?
If you use Hanna ULR Phosphate kit HI-774, please keep in mind, that kit's error range is +/- 0.02 ppm +/- 5% of reading.
I would raise the phosphate a little bit if I was you.

Flow is medium. That is the kit I'm using for phos. How would one raise phos?
