Looks to be hyper-melanization which may have come from coral sting such as anemone or other coral such as hammer, torch or similar. Mine got stung by elegance coral. You dont need to treat with anything as it will self heal and allowing the clown to become accustomed to the sting from host

Other would be a small wound healing over but appears as a sting
Unless these hydroids or whatever they are on the snail could’ve caused it I just noticed them for the first time tonight, they come out of a hole in the snails shell

It could have been spiked by that urchin - granulation tissue that fish develop in response to an injury is often dark in color.
I don't see any signs of full blown disease with this fish, it looks good.
