Discoloration of Favia - Guidance Needed

I am running into an issue with one of my favia corals and looking for some help. The color appears to have changed to gray in some spots. Is this bleaching or death? Any advice on if this favia be saved? My water parameters are as follows:

Salinity: 1.025
Temperature: 78.0F
Nitrates: ~10 ppm
Phosphates 0.02 ppm
dKH: 7
Calcium: 440 ppm
pH: 8.2

I have had it on a frag rack on the side of the tank out of intense lighting and under low/medium flow. All the other corals in the tank are thriving and are growing fast. There are no corals nearby that could have stung it.

It might like a little dirtier water.
