So, does this ever actually happen? I'm thinking about getting a leopard wrasse in the future, and I've always been told that you should place your rocks directly on the glass, then add the sand around them for stability.

But wouldn't that create a potential hazard for sand-burying wrasses? If there's a rock ledge hidden just beneath the sand, couldn't a wrasse accidentally hit its head when diving into the sand after getting startled? Like in this example? Could that seriously injure or even kill the fish?

I've never seen this happen, but that is not to say that it couldn't happen. I just think the chances are very low. Generally speaking whenever I see wrasses burrow in the sand, whether it's to hide or to go to bed they tend to choose a spot that has no direct obstacles right in front of where they are burrowing. Plus the alternative would be if there was a big enough gap of sand between the rock and the bottom of the tank the rock could fall in and potentially topple the structure which can cause a lot of damage.
