Fairy wrasse needs help

Hi All

Since morning today the Fairy Wrasse is not behaving normally, it is lethargic and mostly lying on its sides. On closer look I found some rough patches close to its right side.

All parameters are normal except Nitrate is 20 ppm

Is this something to worry about? What should I do?

Not sure I am seeing this since morning today. The Doctor wrasse picks on it sometimes. I have kept it in a breeders cage now.
Any medicine that I can try?

How long have you had this fish? What other fish are in with it?

I have her for last 3 months.
After she was lying on her sides the Damsels and 6 Lines was poking her until I intervened and separated the fish. It is still lying on its side in the mesh cage with heavy breathing. I have added Melafix to the tank and have kept the Skimmer off.

Doctor wrasse
White Belly Yellow Wrasse
6 Line Wrasse
Fire Goby
Neon Damsel
Scoopas
Powder Brown Tang
Yellow Tail Chromis

The Yellow Wrasse was added last about 15 days ago. Tank is about 220 litres and 100 litres Sump.

Melafix won't help with this. You could try a freshwater dip, mostly to look for flukes so you know what to treat the other fish. At this point, the FW dip will likely prove fatal, but so will not doing one I'm afraid.
