Filling out forms at Doctor's office.

Why is it that when you go to a doctor for the first time and they give you this long form to fill out; they ask for your date of birth and then one line down ask for your age? Are they trying to catch you in a lie? If you ask the lady at the desk why they ask for both she just shrugs her shoulders like she never noticed it.

I thought the same thing at the eye doctor the other day. I had three separate pages that asked for dob, name, and address.

This is an honest question. It happened to me Monday. It happened to me at another doctor's office a year or so ago.

They want your date of birth and then ask you how old you are. I was wondering if they were going to send me a birthday card.

I think the people who design these forms have taken input from several people in different departments who ask different things and they try to put them all on the same form.

If I didn't give them all those numbers they were wanting at the hospital they wouldn't have removed my gall bladder!

They usually arrange files by first two last initials, and dob. They ask age can nobody wants to do the math I guess.
