Frog Spawn Suddenly Retracting. Dying?

I have a frogspawn coral in my tank for about 3 weeks. Was doing well. Yesterday noticed that one of the heads retracted and shriveled and the other head not looking great. All my other coral look good. Water parameters are Phosphate 0, nitrates 10, Alk 10, salinity 1.023, vacuum at 425, ph at 8.3. I am thinking of trying a different location in tank. Should I remove the affected head. Any ideas?

That head is gone. Need a little phosphate and maybe bump salinity up to .025.. lighting and or flow are the other requirements so a move may help.
