Goniopora polyp bailout or Something else?

Hi reefers,

I have had this goniopora for about 5 months now, and it has been doing really well.
It is fully extended during the day and the polyps has almost doubled in size since adding it to my tank.

But I noticed last week a head of the goniopora floating in my tank. And today I found two more on my sand.
Noting has changed in my tank and all parameters are within reason. The goni is still fully extended.
It is currently on my sand bed with medium to low flow. No clown fish are hosting it and I can't see anything that could nip off piece of the coral.

I spot feed twice a week.
And I dose a bit of iron and manganese weekly as I am running a fuge.

Tank only has LPS and I do 20% water changes every two weeks.

So I am not sure if something is up with the goni?

Parameters:
Temp : 25.5C
Alk : 7.8
Nitrates : 2-4
Phosphates : 0.04
Salinity : 34ppt
Cal : 480
Mag : 1500

I have a 40 gal.
Livestock:
Pair of percula clowns
Pair of fire fish
Peppermint shrimp
Longspine urchin
Tiger conch

But none of them were close to that coral
I could not see any pests, but not sure if something could be hiding in the goni skeleton.

With that said the 3 polyps that fell off have found spots on my rock work and they thus far not yet shrivelled.

I want to say I read a thread on here a few months ago where someone’s goniopora dropped a few polyps. I’m pretty sure their last post was that it was surviving.
