Hammer coral lost another head

My hammer coral just lost another head and now I am really worried about it. All parameters are good and third head is also on its way out. Yesterday, it was looking fine, but today another head is gone. I also see some holes in the hammer. Could it be that my snails are damaging the hammer coral?
 
My hammer has been receded for months I haven’t lost any heads yet. I did pull it out today and dip it and found 2 tiny snails hiding out between the 4 heads. Hope that’s all it was of why it’s been closed up for so long.

How long have you had the hammer? Can you share more info please? Parameters, tank age, lighting, flow? A full tank pic always helps.
 
Alk is 9.4, salinity is 1.026, nitrates are 20, phosphates are 0.05, calcium is 460, magnesium is 1450. Tank is 1 year and 3 months old
