Hammer coral suddenly lost a polyp

Hi, Today, when I looked at my tank, a polyp of my hammer was just gone. No sign of polyp bailout or BJD. It just disappeared. My anemone also split and I am worried that something is off in my tank. Could anyone please help me? Thank you. Also, all parameters all good.

Share water parameters please. Most likely something changed rapidly, could be nitrate or phosphate or both.
 
Do you have a peppermint shrimp?
 
What is the fish laying next to it

What other fish are in the tank
 
Yeah, I don’t know what is happening. My nitrates are at 15 and phosphates are 0.05. I took a picture of the anemone because I think the splitting was caused by stress. No, it is not next to the hammer. The acan has been closed since two days ago, but that is most likely because of the anemone, which I am looking to remove.
 
What are you dosing and what is the alkalinity? How are you testing?
