Hammer help!

I’ve had this hammer since November and it was doing great up until about 2-3 weeks ago. It’s been slowly reseeding and I can’t figure out why. All my levels are testing normal and nothing has changed with my water changed schedule, lighting ect.. any ideas on what could be causing it? Thanks guys!
