Hammer issue?

I've had this wall hammer for 3 or 4 months now. He seems to be doing very well. When I went to feed him today it looks like he might be having skeleton issues. Is this growth or something I need to be worried about? I see he has a brittle starfish hanging out under him, that wont cause him any issues will it?

If on the sand, get it off the sand . Moderate light and water flow
