That's a goner man, you can see the polyp bailout...
I'd try to help you out more, but it seems you've been having problems with your tanks, and you've been reading and rectifying poor husbandry, so you're already on the right track

^ Exactly.

The problems of over stocking or stocking a tank too quickly are only going to be over come with time and the establishment of a newer system. You're seeing the classic symptoms of what we use to call "new tank syndrome" there is no magical cure unfortunately, only time and chemistry maintenance and husbandry improvements will make the difference.
