HELP!! Bubble Tip Anemone not looking Good

So I bought a green bubble tip about 3 weeks ago and it did fine for about 2.5 weeks but the last week or so its walked around and been in this one tight crevice for about 5 days and half the tentacles are shriveled up and look all torn, from what I can see the mouth is open not huge but it is not very tight either my parameters are in check and I am very nervous it will die any advice is greatly appreciated.

Yes I feed it finely chopped raw shrimp and squid it seems to capture the food and move it in but I cannot tell if it digests the food.
