Not looking good.

How long have you had it?

How is everything else doing?

Have you noticed any polyp extension in the previous week, or has it been retracted the whole time?

Did you change anything in your tank at the time you noticed the coral start to decline?

Had it for 5+ years . It started as a 2 inch frag. I have sps and LPS everything else is doing great . Sps have lots of polyp extension. Just this one toadstool is looking bad. This piece used to be over 12” when it was happy. Any ideas or advice? It had full polyp extension a week ago.

has a coral perhaps fallen on it? Add any new fishes or inverts that might be picking at it?

It's still alive and leave it alone. Toadstools will periodicly close up and it's best to be patient. Messing with it can just be a source of stress for it which would only motivate it to stay closed longer.
