It is impossible for water quality to be "good" when ammonia is present.

Ammonia burns the gills, affecting how they can breathe.

With you having a lion fish, or any predatory fish. Usually a lot of waste is involved. What food are you feeding it? And how much?
Are you keeping up with water changes? (Is always do more than normal changes in my aggressive tank) instead of once a week, twice.
Also, are you using a Protein Skimmer?
And if your tank wasn’t cycled, keeping fish with heavy bioloads will be a challenge. How many other fish and how big is the tank?
I apologize about all the questions, but you have limited info.
The responses about Ammonia are correct, but want to see why the ammonia to truly solve the issue.
