Help! Toadstool with a hole

Hi this morning i woke up to see that my toadstool has a hole in the head. He was complete yesterday, and opening big like always. I just have two clowns, watchman gobby, snails and several red/blue leg hermit, same setup for over 6 month now. All other corals doing fine. What can this be? Thanks

Not unusual. Generally due to shedding or presence of a worm whereas shedding is the most likely
