Help!!! Why has my lps melting

Iv had a golden torch for about 3 months. And last week I lost a head out if no where. Since then I lost a second head. Iv dipped the coral once. But now I noticed my front spawn is staring to melt and half the head is dead. My phosphates are really high. I added phosguard by seachem. Really don't wanna lost any coral

Sounds like you answered your own question. High phosphates. How high? How olds your tank?

The tank is about a year. It was a 32 gallon but about 3 months ago I upgraded to a 65 cube. All same rock but added some. They are at .75. I just tested
 
phosguard should be used as a preventive not as a solution just so you know also if they have been that high for awhile now then plan to do several waterchanges a week as your rocks will have built up a large amount that will start leaching into the water once you drop the amount in the water which will make it rise back to high levels rather fast I would test every day for the next few weeks at minimum and whenever they get above the threshold imidiatly do another water change to bring them back down and continue to do so untill they stay at an acceptable level then change your what you do as your cleanjng prosses to keep them from ever getting that high again weather its the addition of phosguard or more waterchanges or changing your feeding habbits whatever helps to keep them from ever climbing that high again
 
Also the waterchanges only only a short term solution any idea why they got that high in the first place?

I ask cuz I want to try and help you find a way to keep them from climb again and killing any and or everything
