Hole in coral

Hey,
My Mom added a coral maybe a month ago to her tank and we started noticing these white spots and now it turned into a hole through it. Can anybody give me insight on what could be causing this or how to fix the problem?


 
Mind giving us some background? Age of tank, current parameters, lighting used and settings? Some white light pics may also help out.
 
You didn't even mention what kind of coral? Need more info and pictures please.
