Youre facing ich and needs to be treated in quarantine at your soonest. Best treatment would be coppersafe or general cure with coppersafe my choice at therapuetic level 2.25-2.5 monitored with a reliable copper test kit such as Hanna brand, not Api
Also monitor ammonia level with a reliable test kit during treatment and would not be a bad idea to add an air stone for added oxygen during treatment
You will also need to leave display tank FishLess (Fallow) for at least 6-8 weeks to assure the cysts and trophants have completed their life cycle without a host and have died off

The spots on the tang are mixed sizes - some may just be mucus, others could be ich. The cardinals may have ich, but it may be too soon to tell for sure.
TTM does have a fairly high failure rate because the timing is so complex. 21 days is a bit short for a copper treatment and depending on the dose, it could have let ich through also.
What other fish have potentially been exposed in your DT?

So I have 3, wrasses, 2 clowns, 1 royal gramma and a tomini tang and they are all looking good. I also have a yellow tang that has a few of the mucus looking things on its tail. The story behind the disease is that I went away for the past three month on a work related trip, and a house sitter with no reefing experience was looking after my tank. I came back to find all of my sps dead and everything covered in cyano. Given that I qt’ed everything going in, I am not sure if the fish really have ich, or some kind of infection related to poor water quality and cyano outbreak. Would appreciate your thoughts on differentiating between these two possibilities ? Thanks Jay!

Without doing a skin scrape under a microscope it isn’t always easy to ID ich from other white spots on fish. Generally with ich, the spots come and go at first, going away then returning in new locations every few days. Other white spots on fish tend to be more stable - staying in the same location for days at a time. Also, look for secondary symptoms - scratching, hovering, not eating well or breathing fast. Those all tend to rule out white spots being something benign.
