Is my Rock Flower Nem Dead?

Added him a few weeks ago and he disappeared. Today I noticed him at the bottom of the tank. I feel he may have moved a few times though. Thoughts?

It indeed doesn't appear to be in the best condition... is it attached to a rock or just free at the sand?

Appears free in the sand
yea that's not a good sign at all. Can we get a clearer picture? You should put it in a low flow area/basket with rubbles for it to attach to at this point
