Is this a new polyp growing???

It's possible. To me it looks like a vermetid snail. Does it move with the flow, or is it more rigid? White light pics or video help a lot.

I could be wrong and need to pay more attention to my corals, but doesn’t a new head/polyp generally grow from an area that has flesh?

That is growing below the flesh line and looks like a vermitid. Maybe the thing right above it is a vermitid as well.

Generally, yes. I have seen both but typically it's within the flesh band.

Here are some new polyps growing on my hammer. The hammer was being attacked by a peppermint shrimp, so it was receding and it was easy to see the guys.

Nice pics. I need to pay more attention to the little stuff like this. I generally look at my corals…yup they look healthy and move on to whatever the next task is.

It's all a learning process, and the cool thing about r2r is, we can learn from each other's mistakes. Obviously you care and asked the right questions. That's always a positive thing.

I put the peppermint shrimp in my wife's tank and the hammer is recovering nicely.

That is also a vermetid tube above it. I’ve had success with a combo of manual removal and bumblebee snails.
