He has been in the tank approximately a month and a half there was one new addition a grouper but is no longer with us

That appears to be ick to me as well. A very bad case of it. I would recommend QT for all the fish in the tank and leave the display tank fallow for 76 days. What all fish do you have? Depending on the types of fish, copper will probably be your best bet and I would go with Coppersafe or another chelated copper. It's easier on the fish than cupramine usually is.

All that's in the display tank is that tanning and to clown fish just set up a QT today going to put them in tomorrow

It's probably ich, but I would treat all fish with either copper or CP just in case it's actually velvet.
