Just lost a cardinal into a VorTech MP40

This makes me really sad... I just found one of my banggai cardinals draped across the outside of my MP40, dead. It looks like he was sucked onto the surface and suffocated. So this actually happened about a month ago to a second banggai that I had. Both seemed healthy beforehand. My other fish seem fine and happy.

I’m really sad about this. Is this a known thing? Is there anything I can do to keep this from happening again?
 
I have 5 pajama's and 5 MP60's @ about 30 to 50 %. Never had an issue. I use the foam covers. My shrimp even eat the stuff off the covers.
 
I lost a beautiful male Hawaiian flame wrasse to an MP60 once. That sucked
