It looks like he succumbed to age as the other inhabitants and parameters are fine.
I'm looking for something to replace this blenny. I have a leopard wrasse, yellow tang, blue tang, 2 clowns, and a cleaner shrimp. Any suggestions. Everything has been stable for over 2 years without any casualties.

A Molly miller blenny is a great replacement. They are very similar in appearance, a little smaller, possibly a little less aggressive, but awesome antics. They offer an added bonus which the lawnmower didn't, they eat baby aiptasia, and small pests like worms and such. Plus they are a riot to watch in a bed of palys. Sorry about your loss, you will find a good replacement I'm sure.
