Last couple orders of fish from LA said they would ship from California but when they arrived they actually came from the Rhinelander WI facility. Does this mean the fish were QT'd like Divers Den livestock?

Neither truly QT.
WI will observe for a while, and treat as necessary. However, once done treated, they're often dumped back in with the "general population" and are free to pickup parasites, etc again.
CA (Quality Marine) just holds fish in slightly copper'd water, IIRC, and just ships fish that look decent.
If it came from WI, then it's from the DD facility.
