LT LEATHERY ANEMONE , Not looking good?

My LT leathery anemone was doing fine until 2 weeks ago he started moving around in the tank. His foot looks OK , he is shriveled up and the hosting clowns don't like it. So he moved again and stuck his foot to the underside of a rock with his tentacles laying on the sand. . now he looks like hes getting ready to move again. the lighting has not changed , the tank parems are OK , except the phosphates were a little high, I did a water change today. He is a least 2 years old. I wonder whats up with him. he used to be the most beautiful anemone in the tank? I also have 3 rbta's that have been settled for almost a year now. I don't think LT Leathers split like a RBTA. any comments?:squigglemouth:
 
What are the actual numbers for your parameters --- "ok" doesn't tell me much.
How deep is your sandbed?
What do you have for flow?
What lights do you have?
