Mushroom pushing it's guts out?

So, what's going on here? I've had this in my tank since 05/18 and it started off okay, then slowly started to open it's mouth and never close, until it's just sitting there gaping like so. My tank parameters are normal and I have other mushrooms in my tank, as well as SPS and other LPS like frogspawn, hammers, zoas and acans. Everything besides this mushroom in my tank are doing good.

Also, can anyone help me identify this mushroom? I bought it at a show and cant remember the name of it.

Any other coral near it ?
 
Closest coral is a yellow Aussie hammer and a couple of zoas, but they're more than 6 inches away.
