Looks like an acropora nasuta. Med to high light and medium to high flow, like most acros.

Hes probably gone tbh. In my experience that would end up being a water quality issue.

If there's any good tissue remaining then yes, frag it. Trying to determine why it's RTN is the tricky part. Check water quality, swings in chemistry, even rusty bands in the sump. I just sent through that.
