Came home to him laying on sand bed while heavy breathing. Here are my parameters. Sadly he didnt make it. What can cause this?

More info would help.
How long has your tank been running, did you test ammonia, how long have you had the fish ...

Yes once I seen him on the sand bed I immediately did all my parameters testing. Tank been running for 2 months and I had pair of clowns for 1 month. Other clown look totally fine. I also have a cleaner shrimp that look fine as well

It's possible it was infected. There are many diseases which can cause this.
Velvet and Brook are both very quick at killing fish.
