Powder blue tang death

I have had a powder blue tang for over 9 months. He was king of the 125 gallon tank and swam freely. He was fat and eating well. Loved the seaweed and garlic. Today when I got home from work he was dead stuck to the power head. Do you think it was a freak accident or did he die of something.

Everything in the tank looks good

Nitrites 0
Nitrates 4
Phosphates 1.12
Hardness 10.1
Salinity 33ppt

I was bringing the salinity yo for the last two weeks from 25 to 33 and changing 5 gallons of water every other day. I would think that would have oxygenated the water well and was a daily 5% water change.

Any thoughts?

Clean up crew three weeks ago snails crabs and flower anemone. Sunny D coral two weeks ago. He was daring my long tentacle sea anemone stealing food from him.

But no new chemicals. Added GFO two weeks ago. First time. And daily water changes of 5 gallons to raise salinity not more than 1 PPT per two days. .

Could have been anything. Was it thin, any observable markings, etc. A healthy fish doesn’t usually get pinned by a power head.

That’s what is weird. Super fat swam like crazy back and forth the tank. Was king of the tank, no one messed with him. The only markings I saw were on his body from the Powerhead. I really wonder is if he tried to steal from the long tentacle anemone
