How do I know just when to trim back the algae that is growing in the fuge? Is it a matter of flow, a matter of space, or simply a matter of keeping it healthy by getting rid of the older growth?

One and two. Sometimes three. :)Rarely do I have to get rid of(ro have any) unhealthy pieces, unless they get crowded out.
