I am very interested in doing this mount of T5 Hybrid connected to a RMS mount. Wanted to look for some assistance in trying this out.

This is also what im looking to do. But the instructions on the linked thread are missing what he used for parts and how it was exactly put together.

I am surprised that no one makes this. Is it a concern with weight? You can add quad reefbrites to the Radions, why not T5s?
